Vardenafil is naturally metabolized by liver enzymes (CYP3A4), and a small part of it is cleared from the body via renal system. For this reason, co-administration with potent CYP3A4 inhibitors, such as ritonavir, indinavir, clarithromycin, ketoconazole, itraconazole, etc., may result in a significant increase in vardenafil plasma levels. This, in turn, can lead to low blood pressure and prolonged erections. Long-lasting erections (more than 4 hours) can be dangerous to penile tissues and, if not treated properly, may end in permanent impotence.
PDE5 inhibitors are highly interactive with alpha-blockers, as they are likely to have an additive hypotensive effect. If you are undergoing a treatment course that involves alpha-blocking therapy, do not take vardenafil without talking to your physician. If you are taking alpha-blockers and still think you cannot do without vardenafil, your doctor will advise you to start with the lowest possible dose and stop taking it immediately if any side effects occur.
Concomitant use of vardenafil with organic nitrates is not advisable either, because these drugs have a strong hypotensive effect and therefore there is a great risk of serious blood pressure fluctuations. Erectile dysfunction medicines must be used very cautiously if you have hepatic impairment or renal insufficiency. As a rule, mild cases do not require any specific dosing, but if you have a severe case of impairment, please, ask you physician for advice.
